To the best of my knowledge, my children are in good physical condition and fully able to participate in this course. I understand that accidents can happen and that there is a risk of injury when participating in martial arts, which is in some instances a contact oriented activity.  I voluntarily assume full responsibility for any risks or loss, property damage, or personal injury, including death, that may be sustained by my children. I hereby release, waive, discharge, and covenant not to sue, Master Booe, Master Booe’s Karate Kidz, inc, their officers, servants, agents, and employees. I have read and agree to this waiver. I agree that photos or videos of myself and children can be obtained by Master Booe’s Karate Kidz and Master Booe for promotional purposes.
